Hospice Care Plus, Inc., a non-profit organization located in Berea, Kentucky, has provided quality end-of-life care
to residents in our community since 1981. Hospice Care Plus is a Medicare-certified, state-licensed health care provider. Hospice
Care Plus has been accredited by the Joint Commission of Accreditation of Healthcare Organizations (JCAHO) since 1997.
Hospice Care Plus cares for more than five hundred patients and families annually and provides services to all patients regardless of
their ability to pay. Our organization provides physical, emotional, and spiritual care to patients with a terminal illness and their
families residing in the counties of Estill, Jackson, Lee, Madison, Owsley and Rockcastle.
Currently, end-of-life care provided by Hospice Care Plus is given primarily in the home of the patient and family. Hospice Care Plus
also has contracts with nursing facilities in our six-county service region which allows us to provide hospice care for nursing home
residents who desire it.
In addition to caring for patients with a terminal condition, Hospice Care Plus also offers
bereavement services and support to family
members of hospice patients and to the community-at-large. A
palliative care program, Palliative Care Plus, was added in 2001 in order
to offer home-based support services to those with a life-limiting condition who either do not qualify for or have not chosen hospice
care. Nurses, social workers, nurse aides, chaplains and trained volunteers work as a team to provide the best palliative care possible.
The team focuses on providing comfort and pain relief when a cure is not possible. The goal is to help each person live as fully as
possible and to help the family and loved ones participate in the person's care.
