    One-year experience and Kentucky nursing license required
  • DESCRIPTION: The Registered Nurse (RN) performs nursing care, teaches family members basic and technical skills, and carries out the plan of care to meet identified physical, psychosocial and other needs. The RN is to oversee care provided by the Nursing Assistant to ensure that patient care needs are being met. The RN serves as an active member of the Interdisciplinary Team and the nursing discipline team. The RN is expected to help ensure around-the-clock nursing care is provided by being available to help cover any vacancies in the schedule. The Primary Nurse is also responsible for facilitating team meetings, maintaining the medication profile and coordinating care across settings

  • DESCRIPTION: The Healthcare Community Liaison (HCL) is responsible for day-to-day marketing visits to professional referral sources. The HCL works closely with the Community Outreach and Admissions teams to generate referrals, share information in order to convert referrals to admissions, and improve customer service to, and relationships with, referral sources.

  • DESCRIPTION: The Referral Specialist (RS) is the first and/or primary point of contact for potential patients, caregivers, and referral sources and is responsible for meeting their needs when they contact Hospice Care Plus. The RS takes all referral and inquiry calls, completes referrals, and schedules admissions to hospice and palliative care in conjunction with Patient Care Coordinators. The RS builds relationships with discharge planners, physicians, nursing facilities and assisted living facilities, and works closely with the Healthcare Community Liaison (HCL) and Outreach/Admissions nurses to nurture those relationships. In addition to the primary responsibility of customer service for referral sources and potential clients, the RS assists with medical records, orders durable medical equipment, and completes other clerical tasks necessary to complete or facilitate a referral.
