Hospice Care Plus provides services to meet the emotional, spiritual and physical needs of terminally ill patients and their families. Hospice puts the family and patient at the center of care, and by working with physicians and caregivers, provides compassionate care for individuals nearing the end of life.

Our Services Include:

  • 24-hour nursing support for pain and symptom management
  • a social worker to coordinate community resources, provide emotional support, assistance with insurance and bereavement care
  • a chaplain to assist the patient and family with spiritual concerns
  • a nursing assistant for personal care needs and light housekeeping
  • a hospice pharmacist to help address medication needs
  • medications, medical equipment and supplies delivered to patient's home
  • oversight of care by patient's physician and a hospice
    medical director
