Palliative Care Plus (PCP) is a primarily free program offered to the
communities served by Hospice Care Plus to patients suffering from a long-term illness,
yet not quite appropriate for hospice care. The PCP Team provides a nurse, social worker,
physician and a volunteer to help you get the resources you need to cope with your illness
in the comfort of your home. Coping with a long-term illness can be difficult. Palliative Care Plus helps to:
control your pain
ensure you are accessing all of the community resources available to you
arrange for in-home support for light errands, companionship or other in-home needs
support you and your family
Palliative Care Plus can help if you:
have a progressive, chronic illness or other life-limiting diseases
and need assistance navigating the health care system, or
are still seeking curative treatment, but need in home support to help manage your condition

Can I receive Palliative Care even if I am still pursuing aggressive treatments, such as chemotherapy? Yes. In fact, PCP services may be particularly useful to you in this situation.
If I am a home health patient, do I qualify for PCP? Yes. The PCP team can make home visits to you and your family, even if you are currently being seen by home health.
What can the PCP team do to help me? The team supplements your care by serving as a liasion between your physician(s) and community resources. The Team helps ensure pain and symptom control and offers practical support to your family.
